Data Sharing Agreement

MYSSS CARD DATA SHARING CONSENT AND UNDERTAKING 

  1. I certify that all information provided in connection with my MySSS Card application is true, complete, and correct. I understand that I shall be held liable under all applicable laws for any false information, misrepresentation, or fraudulent declarations made in this application.
  1. I hereby authorize and give my explicit consent to the Social Security System (SSS) and its partner Participating Financial Institutions (PFIs) to collect, use, store, share, and process my personal data, including any sensitive personal information submitted, solely for the purposes of processing my MySSS Card application.
  1. I acknowledge and trust that SSS and its partner PFIs shall maintain the confidentiality and security of all personal information provided, employing appropriate organizational, physical, and technical measures, in accordance with Section 24(c) of the Social Security Act of 2018 (R.A. No. 11199) and the Data Privacy Act of 2012 (R.A. No. 10173). 

I understand that SSS and its partner PFIs shall not disclose my personal information to any third party unless authorized by me or required by law. 

I authorize SSS to share my data with my chosen PFI and its service providers for purposes of MySSS Card processing, replacement application, production, and issuance, account maintenance for the crediting of SSS proceeds, monitoring the status of card delivery and release, and providing assistance in completing my application by reaching out to me through my registered contact information. I also consent to the sharing of my account number by the PFI with SSS for the crediting of applicable SSS benefits, loans, refunds, and proceeds.

  1.  I understand that my SSS Pay Card application is based on my current SSS records; therefore, I warrant that I will not initiate any data amendments with the SSS until the card has been successfully delivered and received..
  1. I understand that my existing/current SSS-issued card will be cancelled or become inactive in SSS records once my MySSS Card replacement application is accepted by RCBC DiskarTech.
  1. I agree and undertake to immediately and permanently destroy my old or replaced SSS-issued card upon receipt of my new MySSS Card. Accordingly, I agree to hold the SSS and/or my selected PFI free and exempt from any and all liability, losses, damages, and/or claims, including any action, suit, or proceeding, as well as other expenses incurred in connection therewith, resulting directly or indirectly from or arising out of, or in connection with the non-destruction and/or use of my old/replaced SSS-issued card.

I voluntarily agree to all the terms stated above and give my informed consent to proceed with my MySSS Card replacement application, including the processing of my personal and sensitive personal information.
